Some care leavers grow up in foster families. The project is gathering insights into how foster children transition into independent living because to date the transition from foster care to self-reliance has received little attention. It is developing the first training modules for professionals and foster parents as well as support measures for the effective empowerment of care leavers in the transition phase. The partner is paying particular attention to the needs of care leavers and foster parents, as well as to those of other relevant stakeholders. The partner is developing and describing a transition model with support measures that is being prepared for implementation in selected regions of Germany and a nationwide rollout in the long term. Subsequently, the model will be implemented via a future project.
