Toolbox Leaving Care

This project creates a bridge between the time when a care leaver must prepare to withdraw from the framework of youth welfare and begin the next steps after leaving state-funded accommodation. The Toolbox Leaving Care project is developing tools to support these care leavers in various areas as they transition to the next stage in their lives. These tools are being developed in the pilot phase and tested in practice. Care leavers and other users are involved in this development process so that they attain a high level of usability for the target group.

By running this project, the Toolbox Leaving Care contributes to the structured and systematic processing of the challenges care leavers face during their time of transition. It helps them and their caregivers to actively work together through the steps of becoming independent at an early stage.
